W-9 Requests:

Departments on campus frequently receive requests for the University’s tax identification number from new customers.  If the customer makes a request by phone, email or by mail with a regular Form W-9, click HERE. Include company name and address, contact person's name and fax number (if applicable)  and I will get a Form W-9 out to them.

If the customer provides an alternate Form W-9, or has a section on their new vendor form for the W-9 information, please forward the form to the Tax office, Old Main Room 401 for completion.  Please include a note with your department information and whether the form should be returned to you or sent directly to the requestor.
