Submitting a Code of Conduct Complaint Against a Student
Members of the University community of student, faulty, or staff may file a complaint against any student or student organization suspected of violating the Student Code of Conduct ("Code"). A complaint shall be in writing and directed to the Dean of Students or a designee responsible for the administration of the student discipline under the Code. Written complaints may be submitted in person, by US mail, e-mail, FAX, or by utilizing the DOS on-line submission form.
To initiate a complaint, please complete the following form. The complaint is not an anonymous submission, and may be shared with accused students and others during student judicial proceedings.
