Report all accidents and incidents promptly.

Note that in some situations multiple forms are required.

A. For all employee work-related injuries and illnesses the "Workers Compensation Report of Injury" form, along with the accident follow-up form, must be completed.  These forms are located on the Human Resources web page (WyoWeb logon required):

If the work related injury meets criteria B  below the "UW First Report of Accident/Incident" must also be completed.

B. For ALL incidents/accidents (including employee injuries) involving:

• a motor vehicle being used for university business,
• damage to university property, or
• injury to a third party,

the "UW First Report of Accident/Incident" must be completed. This form is located on the Risk Management web page:

C. Student and visitor incidents or accidents are reported using the "UW First Report of Accident/Incident" form located on the Risk Management web page:

Once completed and submitted, copies of the forms are sent to the Environmental Health and Safety Department for review, tracking and possible follow-up. EHS uses the information on the workers compensation first report of injury and follow-up forms in completing the required OSHA 300 log of work-related injuries and illnesses.
