Graduate Student Forms

All of the forms listed below and others are available on the Academic Affairs website.  Please contact EMS Office Associate Alice Hamilton with specific questions regarding your form requirements.

Required forms:

  1. Committee Assignment form

  2. Program of Study form

  3. Report on Preliminary Examination for Admission to Candidacy form (PhD students only)

  4. Report on Final Examination form 

  5. Anticipated Graduation Date form 


Optional forms:

  1. Change of Major form (to add, drop or change a major or minor)

  2. Four year post-prelim extension petition (used  to file for an extension if a PhD student hasn't graduated within four years of completing their prelims)

  3. Request for change in graduate program (to make changes to an approved Program of Study)
