Title IV Authorization  [form]

You may choose that federal aid awarded to you is applied to:

  • All UW charges on your account including any minor prior-year charges (under $200)

                          OR

  • Only to UW tuition, fees, and room/board charges.

If you choose this second option, it will not cover student health charges, insurance charges, bookstore charges, IT print quota charges, interest or late charges, etc.  Because your federal aid will not be applied to these other charges, you could receive a refund from your aid, but you may still owe a balance on your account.

If you have any questions regarding Title IV, or would like to cancel or modify your original agreement, please contact the office of Scholarships & Financial Aid by phone at 307-766-2116 or by email FinAid@uwyo.edu

To see what your original choice was, log into WyoWeb, click on Financial Aid, then Award, then Title IV Authorization.

Health Insurance

For all non-international students, the deadline to make any changes to your insurance choice is the add/drop deadline.

For all international students, you are automatically enrolled in the student medical insurance each semester so you must waive the insurance by the add/drop deadline.

Please see Student Medical Insurance for more information.

Exception Procedures

If you are withdrawing from a class after the add/drop date or after the refund period with extenuating circumstances, you may fill out an exception to drop form with the Registrar.

Filling out this exception to drop does not guarantee that your charges will be removed.  The Registrar will review your request and send you notice that it has either been approved or denied.

  • If this is not approved, you may submit an exception to the charges request to fsbo@uwyo.edu.
  • You may submit an exception to the charges to fsbo@uwyo.edu if you have withdrawn from a class or done an all-school withdrawal.
  • In your email please give as many details as possible about your situation, provide valid documentation of your extenuating circumstances, and explain the reasons why you feel the charges should be dropped.

**Before you decide to withdraw or drop classes, please talk with the Financial Aid office!  The money you received for grants, loans, or scholarships may be adjusted or taken back, making you responsible to pay this back to the University.**
