Termination Form Checklist

Termination Form must be submitted to Human Resources prior to employee’s last day of work

Benefited Terminations

  1. Employees may contact the Michelle Holmes with the Human Resources-Benefits Office at 766-2438 for a review of their benefits and to schedule an exit interview.
  2. Departmental keys need to be turned into the department prior to termination.
  3. Parking permits need to be turned in to Parking and Transportation or to Human Resources at the employee’s exit interview.
  4. Terminating employee must report all vacation and sick leave usage to the department and the department needs to post to PeopleSoft HR Time & Labor prior to the employee’s last working day.
  5. Terminating employee should provide forwarding address and phone information to the department to be used on Termination form and the date they want it to be effective.
  6. Department completes the Termination form with benefited employees who accrue vacation completing and signing the Vacation Option section. Department routes through appropriate signing authorities.
  7. Attach a copy of the employee’s resignation letter.
  8. If the employee chooses Terminal Leave, no end date for Budget is needed. Human Resources will add the date after the Terminal Leave is calculated.
  9. If the employee chooses Lump Sum, an end date can be added.
  10. Please advise terminating employee that all computer/email access will be terminated 48 hours after last day worked.
  11. Please note that if the employee does not indicate how they would like their final payment; it will default to a check being mailed.

Non-Benefited Terminations

  1. Terminating employee should provide forwarding address and phone information to the department to be used on Termination form and the date they want it to be effective.
  2. Department completes the Termination form ending where indicated on the form.
  3. Please advise terminating employee that all employment related computer/email access will be terminated 48 hours after last day worked.  Keep in mind students will retain their email address and the associated student access.
  4. Please note that if the employee does not indicate how they would like their final payment; it will default to a check being mailed.
  5. Non-Benefited terminations for hourly employees do not need Appointing Authority signatures and can be emailed directly to HR Records for processing.  Salaried non-benefited termination forms need the appropriate signatures and are routed traditionally.
