Spring CHS Commencement/Nursing Convocation Registration Form

All graduating students are asked to complete a registration form on the College of Health Sciences web site, regardless of participation status for either Commencement or Convocation.

  1. Where is the form?

    • Read questions & answers below for more information.

    • At the bottom of this page, we link to the College of Health Sciences (CHS) 2017 Spring Commencement web page.

    • On that CHS page, you will find the link for the required form located under the Registration heading.

  2. What is the deadline for filling it out?

    Glad you asked!

    April 1, 2017: Initial deadline. 
    Please fill out the information on the form by that time, even if you do not yet have all the answers. Nursing needs this input to aid in planning the nursing convocation.

    May 1, 2017, 5 p.m.: Final deadline.
    If you have new information to add or information that has changed since the April 1 submission, please re-submit the form with that new data by May 1.

  3. Who needs to fill out this form?

    Everyone who plans to graduate this May (2017) or August (2017) needs to fill this form out, regardless of ceremony participation status.

  4. I am not going to participate in either ceremony. Do I have to fill out the form?

    Yes! Regardless of ceremony participation plans, all students are asked to complete the form since it has information we need from you.

  5. Why do I have to fill out the form?

    From the information submitted in the online form...

    • The College of Health Sciences will use the information to plan the Commencement ceremony so that they know who is attending in order to plan for sufficient seating and diploma cases for all participating graduates.

    • The College of Health Sciences will use the information to generate a Commencement Reader Card. Students planning to attend will pick up the card prior to to the ceremony (designated area at the Indoor Practice Field). This preprinted card has your name and hometown listed, along with information required by the photographer.

    • The School of Nursing will use the information to plan the Convocation ceremony so that they know who is attending in order to accommodate their presence.

    • The School of Nursing will use the information to generate a Convocation Reader Card. Students planning to attend will pick up the card prior to the ceremony (as noted on Events page). The information on the card detailing your plans for the future will be read about you at the ceremony. For those NOT participating, we would love to know how you will be using your nursing! Photographers will have card copies as well for correct spelling of your name and contact information for sending individual pictures.

    • The School of Nursing will use the information to generate a listing of updated contact information for your class and forward it to the UW Foundation and Alumni association. It is through their database that the School of Nursing accesses addresses for mailing the annual Alumni Newsletter, quarterly emailed news briefs, etc.

  6. I do not yet have all the information the form requires. I may not know it until April or May. Should I wait and fill it out later?

    Please fill out the form by April 1 even if you do not yet have all the answers. As noted above, nursing needs your input to aid in planning the nursing convocation. You can re-submit the form later when you have more information, but the final deadline for submitting the form is 5:00 p.m. on May 1.
