Consent for Online Enrollment Petition

This form is used to request consent for online enrollment in nursing courses for the following individuals:

1. the RN with a non-nursing baccalaureate degree who is seeking enrollment in bridge courses to meet admission criteria to the Graduate Nurse Educator Program (application packet must be on file with the School of Nursing)

2. the non-degree seeking graduate student seeking enrollment in NURS 5140

NOTES:

• This form must be completed and approved prior to registration in the course. Failure to receive consent will result in administrative withdrawal from that course.

• A separate form is required for each course being petitioned.

• Students will be notified as to the outcome via email.
