Honduras Calendar Order Form

Suggested price per calendar: $25

($5 pays for the actual calendar; the balance of your purchase will go toward building a health clinic for Honduras. Any amount over the $5 per calendar will be tax-deductible. UW Foundation will send you a receipt for that amount.)

Directions:

1. Make out your check to: UW Foundation

2. In the memo section of your check (lower left-hand corner) write: "Honduras Clinic/Calendar"

3. Mail your check to:

4. Support Disbursement
   Upon receiving your funds, Ms. Caldwell will promptly disburse the money to the Foundation and will mail or deliver the calendar(s) to you.

5. Fill out the Form
   Please continue on and fill out the form below so that we have the information we need to reserve the calendars and have them ready to mail upon receipt of your funds. You will be asked to give either your email address or phone number in the form below. Please give at least one so that we can contact you should we have questions.
