Applying for UDSS Services
As a student, it is your responsibility to identify yourself to UDSS as an individual with a disability and to request accommodations. According to the laws protecting individuals from discrimination based on disability, an individual with a disability is someone with a physical or mental impairment that substantially limits a major life activity such as caring for one's self, performing manual tasks, walking, seeing, hearing, eating, sleeping, standing, lifting, bending, speaking, breathing, learning, working, thinking, reading, concentrating, and communicating.
Students wishing to receive accommodation services must register with UDSS. For more information on the application process, please call our office at (307) 766-6189, TTY: (307) 766-3073 or stop by our office in 109 Knight Hall, 8:00-5:00 PM, Monday through Friday, to pick up application materials. You may also set up an in-person or phone appointment with a UDSS coordinator if you have additional questions. We also are happy to provide fact sheets about our services. Please let us know if you need any of the materials in alternative format.
Checklist for Applying and Initiating UDSS Services (4 easy steps):
1. Complete the UDSS Application for Services (link below).

2. Gather and send UDSS documentation of the condition(s) that may be a disability for you. This information will assist UDSS staff determine if a disability is present and to help determine appropriate academic adjustments as well as auxiliary aids and services. Information you share from your own personal experiences may also help with eligibility determination decisions.
The documentation needs to be current enough to explain how the condition(s) affects you now, especially in light of your anticipated need for assistance to accommodate your disability. UDSS has documentation guidelines to help you determine where to locate the appropriate materials. Note: These are guidelines, not strict requirements. The documentation you provide will be evaluated on an individual basis, along with the information you provide in your application for services. Students who need to determine whether they have a condition such as a learning disability, Attention Deficit Disorder, etc. may need to seek out professionals to conduct an assessment. 3. Your application and documentation will be reviewed as soon as possible after receipt. You will be notified by phone or mail whether additional information is needed and/or if you qualify for services. If you do qualify, you will be assigned to a UDSS coordinator and encouraged to schedule an appointment to begin planning for your accommodation needs.
4. Call our main office at (307) 766-6189, TTY: (307) 766-3073 and ask the staff who answer to help you make an appointment to see your coordinator. Please bring your course syllabi or outlines to your first appointment. You and the coordinator may use that information to help determine the details of services tailored to you.
