Do you know of a program that isn't listed in Pathways Plus but should be? Is there a program in Pathways that needs to be updated? Tell us about it!

The above surveys are designed for programs that provide Medicaid Waiver services and those that do not. We have stopped using downloadable forms for updating or adding information because the online surveys are more efficient.
