Person Centered Planning

Person-centered planning is an individualized approach to planning that supports an individual to share his or her desires and goals and to consider different options for support. Each participant in Think College Wyoming will engage in a Person Centered Plan Process to organize and coordinate individualized services.

Goals of the PCP process

  • To identify the student’s own vision of the future and identify steps needed to make that future a reality.
  • To gather input from the student, family members, and others who have a vested interest in the student’s future.
  • To develop an action plan that identifies goals services and supports and the people who will assist the student.

At a PCP Meeting

  1. Everyone brainstorms a list of student‘s unique gifts and talents.
  2. Everyone, including the student, contributes thoughts and ideas about the student’s future.
  3. A big picture plan is formed by going over a list of questions tied to: Career development and employment, academic enrichment, campus and community engagement, independent living, and self-determination
  4. A review of “here and now” status is discussed from step 1.
  5. Suggestions are written that will become the action plan.
  6. Discussion identifies what everyone will need to do next to work toward goals.
  7. People are identified as being responsible for action steps.

After the PCP meeting

  1. Complete the Action Plan and get copies to all participates along with contact information.
  2. Schedule a new PCP if the student’s direction takes a ”new turn
