Transition Across the Community Team (TACT©)

TACT© is a patient-centered program designed to improve quality of life, reduce re-hospitalization, and contain costs for patients who are 18 years of age and older and are considered high-risk. The program provides TACT© Nurse/Care Managers to guide patients as they transition across multiple health care settings.

Successful Components of the TACT© Model

  • Focused, well-coordinated high-risk care management

  • Home visits and subsequent telephone contact for up to 90 days

  • Medication and disease management

  • Coaching, education, monitoring, and self-management

  • Resource referral

  • Supported by Patient-Centered Medical Home (PCMH) and Medical Neighborhood

Training Opportunities

  • Comprehensive training program designed to meet your specific needs and provides CEUs or certificate of completion

    • Sixteen or 8 hour options

    • Individual training modules

    • All training options and modules are free

      • Includes training materials

      • On-site instructor

      • Web-based options

TACT© is powered by the Wyoming Institute of Population Health, a division of Cheyenne Regional Medical Center. Funding for the training program and materials is made available through a 3-year Health Resources and Services Administration (HRSA) grant, Geriatric Workforce Enhancement Program (GWEP), with the University of Wyoming as a sub-recipient. Training is available to all Wyoming PCMH recognized clinics.
