TACT (Transition Across the Community Team Program)

Transition Across the Community Team®


TACT is a patient-centered program designed to improve quality of life, reduce re-hospitalization, and contain costs for patients who are 18 years of age and older and are considered high risk.  The program provides TACT Nurse/Care Managers to guide patients as they transition across multiple health care settings.

Successful Components of TACT Model:

  • Focused, well-coordinated high risk care management

  • Home visits & subsequent telephone contact for up to 90 days

  • Medication and disease management

  • Coaching, education, monitoring and self-management

  • Resource referral

  • Supported by PCMH & Medical Neighborhood

  • Comprehensive training program designed to meet your specific needsCEUs or certificate of completion
    • Sixteen or 8 hour options
    • Individual training modules
    • All training options and modules are free
      • Includes training materials
      • On-site instructor
      • Web based options

TACT is powered by the Wyoming Institute of Population Health, a division of Cheyenne Regional Medical Center.  Funding for the training program and materials is made available through a 3 year Health Resources and Services Administration (HRSA) grant, WyGWEP, where the University of Wyoming is a sub-recipient.  Training is available to all Wyoming PCMH recognized clinics
