Occupational Health

The Office of Laboratory Animal Welfare (OLAW) requires Universities with Public Health Service Funding to have a mandatory occupational health program for all animal users (regardless of funding status or source) as part of their Animal Welfare Assurance.

Each individual participating in University-approved animal work will be expected to submit the following forms via the HIPAA-compliant links below:

  1. Animal Exposure Risk Assessment
  2. Baseline Health Questionnaire
  3. Animal Allergy Screening Questionnaire

Program Highlights

  • Grand Avenue Urgent Care (GAUC) will serve as our contracted Occupational Health Provider
  • Animal users submit screening questionnaires through an online HIPAA-compliant portal for review by GAUC
    • GAUC will contact individuals directly with questions or required follow-up. UW staff will NOT have access to personal health information. Do NOT send UW employees your private health information (PHI)
  • IACUC protocols - PI’s must certify participation in ROAMWyo Animal Oversight for all researchers with more than incidental contact with animals
    • Research Office will confirm this via a list of “cleared” individuals provided by GAUC
    • Individuals will not be approved on new IACUC protocols until “cleared” by GAUC
  • Research Office will cover costs for UW employees/students
  • Approximate Pricing (just an FYI):
    • Each application is $50 plus any required additional services (vaccination, titer etc.)
    • Rabies per injection-$500
    • Rabies booster-$500
    • Provider visit $175
  • Please plan ahead—it could take 2 weeks to receive clearance from GAUC to begin animal work. If you know that you will need vaccinations (e.g. rabies) please plan accordingly and begin this process as soon as possible. This is particularly important for seasonal employees.
    • We do not plan to “halt” research for those already involved in animal work. We will ask that you submit your questionnaires online at your earliest convenience.
