The purpose of this manual is to serve as a resource for the planning, conduct, evaluation, and reporting of WyCOA’s implementation of the Chronic Disease Self-Management Education (CDSME) programs authored by Self-Management Resource Center (SMRC). This implementation is referred to as “Healthy U.”

Healthy U is coordinated by the Wyoming Center on Aging (WyCOA) at the University
of Wyoming and delivered in partnership with the Wyoming Department of Health’s Aging
and Public Health Divisions.
WyCOA is supported by the Health Resources and Services Administration (HRSA) of the
U.S. Department of Health and Human Services (HHS) as part of an award totaling $5,000,000
with 0% financed with nongovernmental sources. The contents are those of the author(s)
