- In the "Gift Designations" section, check on the bubble next to "Click here for a list of designations." Click the down arrow (\/) next to "College of Health Sciences" and then check the box next to "Kinesiology & Health." You may also choose 'Other Fund Not Listed'.
- Click on "Continue" to return to the main screen.
- If you chose 'Other Fund Not Listed' enter your designation in the box below.
- Continue filling out form as directed.
