UW’s Albany Community Health Clinic Receives Patient-Centered Home Recognition

The University of Wyoming’s Albany Community Health Clinic (ACHC), part of the Educational
Health Center of Wyoming (EHCW) in the College of Health Sciences, recently received
certification and recognition from the National Committee for Quality Assurance (NCQA)
as a patient-centered medical home practice.
All UW-operated healthcare clinics within the EHCW now are recognized as patient-centered
medical home practices.
From somewhat humble beginnings in a small clinical space on Laramie’s Sheridan Street
in 2017, the ACHC now occupies a large multi-exam room facility in UW’s Mountain View
Medical Center with numerous medical and behavioral health providers and staff members.
For a medical facility that sees patients, the designation as a patient-centered medical
home practice indicates excellence in primary care with a focus to provide consistent,
quality care tailored to the individual patients and their families.
“Earning patient-centered medical home practice recognition reflects our team’s deep
commitment to providing high-quality, equitable care,” says Sara Dalton, ACHC clinic
director. “We want every patient to leave our clinic feeling heard and empowered with
a clear path forward. This designation is more than a title; it’s a promise to continue
improving the health and well-being of our entire community.”
“All three EHCW clinic sites are now patient-centered medical home practice-certified,
with ACHC joining Wyoming Family Practice in Casper and UW Family Medicine in Cheyenne
in this accomplishment,” says Beth Robitaille, a physician and designated institutional
officer and director of UW’s Department of Graduate Medical Education.
“As dean of the College of Health Sciences, I am proud of the Albany Community Health Clinic and the entire EHCW network for achieving and maintaining NCQA patient-centered medical home recognition across all UW-operated clinic sites,” Patrick Hardigan says. “This designation reflects not only strong clinical performance, but also the intentional integration of education, interprofessional practice and continuous quality improvement within our care delivery model. It is a tangible example of how the college is advancing its mission to expand access to high-quality primary care while preparing the next generation of health professionals through immersive, community-based training environments that directly serve the needs of Wyoming patients and families.”
