FAQ'S

Anyone who wishes to drive any vehicle on UW business must complete one of the following forms no later than DECEMBER 1, 2011 in order to be granted driving privileges. Drivers must meet all qualification requirements set forth in the Official Policy.

 

NOTE: In each form, there is a field labeled 'NOTIFY'. If you're a non-employee, please list the name of your UW contact in this field. They will receive notification if you are NOT qualified to drive. If you ARE a UW employee, this field can be left blank or filled in if your department would like an additional party OTHER THAN OUR SUPERVISOR (i.e. business manager, director, dean) notified if your driving record is NOT satisfactory.

Passengers are not allowed to travel on UW business if they are not directly related to and/or required as part of Official University Business. Rare exceptions will be made at the discretion of the Risk Manager. If you wish to request an exception in order to travel with an unaffiliated passenger, please choose one of the following forms based upon your particular circumstances (one time exception or multi-trip exception) and submit to Risk Management.

Passengers may travel on University Business in a personal vehicle if they sign a waiver (see below). Also, if you choose to travel on University Business in your personal vehicle, please understand that your personal auto insurance will be primary.  

Exception Form submissions must be completed at least 5 working days prior to the planned travel in order to receive consideration.

 

Please complete 1 of the following forms and submit to RISK MANAGEMENT if you plan to travel with a passenger in a UW OWNED VEHICLE:

UW Vehicle One Time Passenger Exception

UW Vehicle Extended Duration Passenger Exception

 

If passengers not related to UW business are traveling on UW business in a PERSONALLY OWNED VEHICLE, one of the following waivers must be signed by each passenger and on file in your department prior to any travel. Departments are responsible for filing the waivers with Risk Management.
