WWAMI Student Health Insurance Information

 

The University of Wyoming (UWYO) Student Medical Insurance program is designed to be an annual policy. Coverage dates are tied to the University of Wyoming academic calendar.

 

The 2017-2018 policy has coverage effective August 29, 2017. Annual coverage begins the first day of fall classes for UWYO. WWAMI students have the option to enroll in the coverage beginning August 1, 2017 if you will be attending classes and coverage is needed before the start of the policy year. The premium will be prorated on a monthly basis.

 

Eligibility: Students must be enrolled medical students through the WWAMI Medical Education Program in order to be eligible. Students must actively attend classes for at least the first 30 days after the date for which the coverage is purchased.

 

Premiums:  All premiums must be paid in full at the time of enrollment in the Student Medical Insurance Program.

  

Enrollment Procedures:

  • The University of Wyoming Student Medical Insurance program is designed to be an annual policy.  Coverage dates are tied to the University of Wyoming academic calendar.  The annual coverage begins the first day of fall classes for UWYO.  In the event WWAMI classes/clerkships begin BEFORE classes at UWYO (as is usually the case), WWAMI students may elect to purchase coverage from the beginning of WWAMI classes.  The premium will be prorated on a monthly basis.

 

  • Student must complete the attached UnitedHealthcare Enrollment Form.  Completed Enrollment Forms and a check or money order payable to the University of Wyoming Risk Management in the amount of the full premium must be received by the WWAMI Office (Laramie) . There is no option to pay with a credit card at this time.  Deadlines are as follows:
    • For annual coverage from 8/29/17 to 8/28/18: Due Date is 8/25/17 
      • Premium due: $ 2,044.00
      • On enrollment form select “Annual Coverage.”  Please enter your W# as your Student ID.

 

    • For annual coverage from 8/17/17 to 8/28/18: Due Date is 8/18/17
      • Premium Due: $2,214 ($2,044 annual premium plus $170 for August)
      • On enrollment form select “Annual Coverage” AND “Monthly.”  Next to monthly write ”August.”  Please enter your W# as your Student ID.

 Please review the enrollment form and verify the first page has all fields completed (including social security number and student ID). UHCSR needs all of the information for enrollment and tax purposes.

    • Please send Enrollment Form and Premium payment (payable to University of Wyoming Risk Management) to the following address with enough time to ensure receipt by the due date stated above:

      • Attn: Brandi Hecker

Any checks made out to the UW WWAMI program, will be returned and a corrected check MUST be made payable to the University of Wyoming Risk Management must be received by the above due dates. No additional time will be allowed for returned checks.

    

  • Students are only eligible to enroll at the beginning of fall classes.  Exceptions can be made when a student has a Qualifying Event (e.g. loss of coverage under parent plan after reaching age of 26, marriage, divorce, etc.).  Proof of qualifying event must accompany Enrollment Form and premium payment. All documentation must be received by the Wyoming WWAMI program office 10 days before the date coverage is to begin.  Students have 30 days after the qualifying event to apply for a University of Wyoming Student Medical Insurance policy. 

 

  • This is an annual policy and is NOT automatically renewed each year.  Students must complete a new Enrollment Form at the beginning of each academic year. If you need a receipt, please include that request with your enrollment form and payment.
