Attention Students: The following form is for work order requests for campus facilities that are NOT the residence halls or UW apartments.
To report issues in the residence halls or UW apartments, please submit a work request using Residence Life Operations' Work Order System.
For urgent requests outside of the residence halls or apartments, please call the UW Operations Service Desk at 766-6225.
