All of the forms listed below and others are available on the Registrar's website.  Please contact the Veterinary Sciences Business Office with specific questions regarding your form requirements.

Required forms:

• Committee Assignment form

• Program of Study form

• Report on Preliminary Examination for Admission to Candidacy form (PhD students only)

• Report on Final Examination form 

• Anticipated Graduation Date form 

Optional forms:

• Change of Major form (to add, drop or change a major or minor)

• Four year post-prelim extension petition (used  to file for an extension if a PhD student hasn't graduated within four years of completing their prelims)

• Request for change in graduate program (to make changes to an approved Program of Study)

• Committee Change Form (to make changes to your Committee)
