Do you know of a program that isn't listed in Connect Wyoming, but should be? Is there a program in our database that needs to be updated? Tell us about it!

• To tell us about a program or service not yet in our database;
• use our Add New Program Form.

• To update information we already have for a program or service;
• use our Program Update Form.

These forms are designed for programs that do and do not provide Medicaid Waiver services. We have stopped using downloadable forms for updating or adding information because the online forms are more efficient.
