Preparing for entry to AFROTC at UW


If you plan to join AFROTC at UW this fall, please complete the following checklist: 

  • Sign up for AIR1010 when registering for classes (and AIR2010 if you are a 3 year student).  You will also need to sign up for Leadership Laboratory on Wednesdays, 3:30 p.m. - 5:30 p.m.   
  • Review the Commander's Welcome Letter for Incoming Cadets.   
  • Complete the online application process (setup a WINGS account) prior to arrival at the detachment. 
  • Wings Application Instructions
  • Apply for AFROTC Online (Link to AFROTC official site)
  • Follow the In-processing Instructions (PDF)
  • Have the In-processing documents listed below ready orientation. 
  • At the very minimum, complete the AFROTC Sport Physical Form.

 

ORIENTATION SCHEDULE OF EVENTS:

1330 - Introduction with Detachment 940 Cadre and Upperclassmen Cadets (Business Building Room 121)
1400 - Enrollment paperwork, document verification (Business Building Room 121)
1500 - Cadets: Meet & greet with Flight Commander; Parents: Q/A with cadre (Business Building Room 121)
~1515 - Transition to BBQ (Washington Park)

The ROTC Required Attachments are listed and linked below.  Download and complete all attachments to bring with you to orientation.  All attachments are in PDF format. 

  1. Enrollment / What to Bring Form
  2. AFROTC Form 28 (Sports Physical) – Must be completed NO EARLIER than 30 July.  May be completed by Family Physician or On-Campus Health Services Office
  3. Drug Demand Release Memo (Urinalysis Testing)
  4. Student Records Release Form
  5. DD Form 2005 (Health Care Records Authorization)
  6. DD Form 93 (Record of Emergency Data)
  7. DD Form 2983 Recruit/Trainee Prohibited Activities Acknowledgment  
  8. AF Form 2030 USAF Drug & Alcohol Abuse Certificate
  9. Wings Enrollment Instructions (Apply to ROTC) – MUST be accomplished PRIOR to orientation!
  10. Immunizations Records
  11. Copy of ACT & SAT score sheets
  12. Copy of Birth Certificate
  13. Social Security Card
  14. Selective Service Card
  15. Mail Release Memo  
  16. AF 4428, Tattoo/Brand/Body Marking Screening/Verification (for applicants with tattoos/brands/markings only)

 

If applicable, also bring the following documents (copies are acceptable):

  • College Transcripts (other than UWyo)
  • High School Transcripts (If AP Courses)
  • Private Pilot’s License
  • JROTC certificate
  • Eagle Scout certificate
  • Girl Scout Gold Award certificate
  • Civil Air Patrol Award certificate
  • DD Fm 785 – Record of Disenrollment from Officer Candidate Training (i.e. AFROTC or Service Academy)
  • DD Fm 4 – Enlistment/Reenlistment (if prior military service)
  • DD Fm 214 – Copy 4 – Certificate of Discharge (if prior military service)

FOIA

The Freedom of Information Act (FOIA) Program is mandated by Congress and applies to all governmental agencies. The program allows the public to request information pertaining to an agency's functions, practices, and future plans. Anyone may request records under the FOIA.
