Annual Wellness Visit Certificate Course
Jan 11 - March 22nd, 2027. Register By: Jan 25th
This course is designed to provide health professionals, such as Nurses, Health Educators,
Health Coaches, and alike, with the knowledge, skills, and abilities to conduct this
in-depth consultative visit and develop an individualized preventive plan of care
as a valuable member of the primary care provider team.

Transitional Care Management Course
Jan 18 - March 15th, 2027. Register By: Feb 1st
Designed to provide health professionals, such as nurses, and primary care clinical
staff, with the knowledge, skills, and abilities to provide Transitional Care Management
(TCM) services to effectively reduce readmissions and to prevent adverse events that
may result during transition, such as medication errors or treatment delays. The elements
of TCM will be outlined in an easy-to-understand format and will provide background
material for the understanding of TCM.

About the InstructorS:
Marcella A Wright, DNP, MS, RN, Assistant Director Care Coordination & LEAN Consulting: Marcella’s nursing career has spanned across 30 years and includes acute care, home health, mental health, renal health, and academia. With a lifelong passion to provide whole person care, consulting within Care Coordination. Her leadership roles have included clinical director, lead positions, and executive teams. Her knowledge and experience span various settings including nursing school classroom, clinical instructor, outpatient care, outpatient hemodialysis, hospitals, and home care. She has been a certified mental health nurse and a certified hemodialysis nurse, has been member of the American Nurses Association, American Nephrology Nurses Association, and the American Academia of Ambulatory Care Nurses. Marcella is also involved in her community and has held multiple leadership positions serving within athletics, agriculture, children, and service.
Faith M Jones, MSN, RN, NEA-BC. Consultant: Faith Jones has worked in a variety of roles in clinical practice, education, management, admin, consulting, and healthcare compliance. In her leadership roles she has been responsible for operational leadership for all clinical functions including multiple nursing specialties, pharmacy, laboratory, imaging, nutrition, therapies, as well as admin functions related to quality & case management, medical staff credentialing, education, and corporate compliance.
