Wyoming's older adults deserve the best care. The Rural Community Linkage Toolkit, developed by the Wyoming Geriatric Workforce Enhancement Program (WyGWEP) at the Wyoming Center on Aging, helps bridge the gap in delivering comprehensive care.

This FREE toolkit provides practical strategies and resources to improve older adults' health and well-being while addressing rural healthcare's unique challenges, including access and resources for managing age-related diseases including dementia and other cognitive impairments.
