Forms

Statement of Work

• Statement of Work - should be utilized for service arrangements valued at $5,000 to $9,999 for which the supplier does not require its own contract - UPDATED 6/20/22

Agreement for Services

• Agreement for Services  - should be utilized for service arrangements valued at $10,000 and above and as necessary in other circumstances - UPDATED - 10/23/15

Addendums

• "Ownership of Work Product" Addendum - should be utilized when the University wants to own the end product, including design/photo contracts - UPDATED 1/1/15
