Vaccine Resources

BACKGROUND

The University of Wyoming has several units that serve Medicaid and Medicare patients. UW’s family medicine residency centers in Casper and Cheyenne, the Albany Community Health Clinic in Laramie, the Speech, Language, and Hearing Clinic, and Student Health Services are included. Employees of these CMS units who choose to receive a COVID-19 vaccination must report and upload proof of their vaccination through the Vaccine Resources Website.  Other outside entities may also require university employees to provide either proof of vaccinations or submit exemptions processed and approved through the University for those Vaccines.   This page provides resources for employees to submit proof of vaccination or Religious and Medical exemption forms to request an exemption.  

  • UPLOAD VACCINATION


    The University of Wyoming utilizes Medicat as a secure resource to upload vaccination information for verification. Links provided below will allow access into the systems as well as step by step procedures for employees to enter their data. Only personnel identified as needing access through their department will have access to the database. 


  • RELIGIOUS EXEMPTION


    Individuals who are requesting a religious accommodation in the University of Wyoming Speech and Hearing Clinic, EHCW (Albany Community Health Clinic, UW Family Medicine Clinics at Casper and Cheyenne) the University Student of Wyoming Health Services and the Student Health Pharmacy should complete this form and return the completed form to the Department of Human Resources.


  • MEDICAL EXEMPTION


    Individuals who are requesting an exemption from the COVID-19 vaccine requirement due to a medical contraindication at the University of Wyoming Speech and Hearing Clinic, EHCW (Albany Community Health Clinic, UW Family Medicine Clinics at Casper and Cheyenne) the University of Wyoming Student Health Services and the Student Health Pharmacy should complete this form and return the completed form to the Department of Human Resources.
