FASI Assessments

What is a FASI Assessment?

The Functional Assessment Standardized Items (FASI) is an evaluation to help determine the services and supports needed by people with disabilities and elderly adults. The Wyoming Department of Health, Home and Community Based Services (HCBS) Section contracted with WIND to research how well the FASI can accurately identify the level of care needed for this population in Wyoming. The FASI assesses the following:

  • Self-Care

  • Mobility

  • Instrumental Activities of Daily Living

  • Assistive Devices for Everyday Activities

  • Living Arrangements

  • Availability of Paid and Unpaid Assistance

  • Behavior and Emotions


WIND will be conducting FASI evaluations starting in September 2023.  Evaluations will be conducted by trained assessors at WIND and will take up to one hour to complete. People currently served by HCBS will be randomly selected to complete the FASI and will receive an email and flyer from WIND. Meet the FASI team!
