Dementia Support Center

WyCOA Dementia Support CenterDOWNLOAD THE Dementia Support Center BROCHURE

The purpose of the Wyoming Center on Aging, Dementia Support Center is to enhance the dementia-capability of our community through education, and provision of home and community-based services. The Dementia Support Center is supported by a grant received by the University of Wyoming Psychology Department and Wyoming Center on Aging.

We offer evidence-based interventions to meet the specific needs of people with dementia, and their family members. We also offer dementia screening for people with intellectual and developmental disabilities (IDD) who may have or be at risk of dementia. Additionally, the Dementia Support Center provides training programs for community members, first responders, health care and service providers to increase their ability to provide dementia-capable care.

  • TCARE®: Tailored Caregiver Assessment & Referral System.  TCARE® care consultants identify caregivers’ risk of stress and burnout and equip family caregivers with the right resources and services based on tailored care plans for every unique caregiving journey.

  • SHARE: Support, Health, Activities, Resources, and Education.  A Care-Planning Program for families facing an early-stage dementia diagnosis.  The early stages of dementia can be an important time to prepare for the changes that lie ahead. The share program empowers persons living with dementia and their families to get the most out of today while planning for tomorrow.  SHARE is an evidence-based intervention that involves both care partners in decision making and planning.

  • Supported Independence/Living Alone with Dementia: Support services to people with dementia who are living alone in the community, or who need additional support, and their family caregivers.

  • Dementia Capable Care of Adults with Intellectual Disabilities and Dementia
