Healthy Heart Ambassador Program Facilitator Training
Do you want to be a Healthy Heart Ambassador Program Facilitator and help educate and support people in your community living with high blood pressure? The Chronic Disease Prevention Program is offering this 3-hour training on September 15th, 9 am to 12 pm.
In this training, you will: Learn all about the Healthy Heart Ambassador program. Learn what the role of Program Facilitator entails. Get time to practice being a Program Facilitator. Receive all necessary program materials. The Healthy Heart Ambassador program is a 4-month program that includes 10-minute consultations with a program facilitator during drop-in office hours, weekly check-ins by phone, email, or text from the program facilitator, and monthly nutrition education seminars. Program participants are asked to attend two personalized consultations per month and the monthly nutrition education seminars. Anyone can become a Healthy Heart Ambassador Program Facilitator!
