Forms required or used by the Office of Scholarships & Financial Aid are located here in one convenient location!

We highly recommend that you use our Secure File Upload to submit most forms to our office. Emailing any form that contains PII (personal identifiable information) could put your information at risk. You may also mail, fax, or submit documents in-person using our Contact Us information at the bottom of the page.

Some forms will require a wet signature (a physical, handwritten signature made with ink) and cannot be submitted online. Please contact our office with any questions you have.

Forms Related to the FAFSA

If you or any of your FAFSA contributors had asset information that was either not provided on the FAFSA or is incomplete, you must submit that information to complete the federal verification process.

If you or any of your FAFSA contributors have made changes to your FAFSA, our office must determine the accuracy of those changes.

 

If there is a discrepancy between your college grade level reported on the FAFSA and your current student status, you must complete this form.

 

If there is conflicting information for you or any of your FAFSA contributors, our office must confirm that information. This usually has to do with the tax filing status of an individual and often occur4s when comparing financial information uploaded from the IRS to anything you manually entered on the FAFSA.

Students can enroll at more than one institution and have their credit hours combined to meet enrollment requirements for financial aid. More information and the form can be found on our Consortium Agreement page.

Students can secure scholarships that are funded by organizations external to UW to help pay your education expenses. More information can be found on our External Scholarship page, and we urge donors to include the form when submitting payments.

The Family Educational Rights and Privacy Act (FERPA) is a federal law that protects the privacy of student education records. A FERPA release can be provided by the student to allow another designee(s) certain rights to those education records, including getting information about their financial aid. If you do not already have an Authorized User(s) set up, you can complete a FERPA Release form.

There are several situations where students can appeal their financial aid. Our Financial Aid Appeals page goes into details about those situations and when each type of appeal form should/could be used.

Occasionally a student may request to have their Federal Tax Information (FTI) from their FAFSA released to a third party. They must provide written consent by using the Student Release of Tax Information form.

Students who receive federal aid must maintain Satisfactory Academic Progress (SAP) eligibility standards that are reviewed each semester. Students not meeting those standards will need to complete a SAP appeal and Academic Plan. If approved, will allow them to continue receiving federal aid.

The Teacher Education Assistance for College and Higher Education (TEACH) Grant Program offers financial assistance to students pursuing a career in Education. To be eligible, students must commit to meet several requirements following graduation from UW. More information can be found on our Federal Aid page.

Students can notify our office of their intent to not attend a current or upcoming semester by completing the Will Not Attend form. If you are a continuing student who has previously received financial aid at UW, please note there may be an impact to your current and future financial aid. Please contact our office with any questions.

Students considering an all school withdrawal should following this process:

  • STEP 1 - Meet with the Dean of Student's office. You can make an appointment in Knight Hall Rm 128 or by calling (307) 766-3296.
  • STEP 2 - Meet with the Office of Scholarships & Financial Aid to discuss the impact of withdrawal on your scholarships, federal financial aid, private loans, or any other types of financial aid.
  • STEP 3 - After steps 1 and 2 are complete, you will be referred to the Registrar's Office to change your academic record for the semester and record your withdrawal. The day your information is entered into the system will be considered y our official withdrawal date from UW.

Verification Forms

Students may be selected for the FAFSA verification process and must submit requested forms and documentation to our office.

Since the family size is based on the number of individuals listed and claimed on the IRS tax return, if the information provided on the FAFSA is not transferred directly from the IRS, or if the applicant updated the family size when presented the opportunity to do so on their FAFSA, the following form(s) is required.

This federal verification process is required by the Department of Education (ED) if you are selected for this type of verification.

Selected students can appear at the Office of Scholarships & Financial Aid in person to present your original, government-issued photo identification for verification, and to complete the form at that time.

Students unable to travel to campus may complete complete the form and then appear in person before a Notary Public who will complete the Notary Certificate of Acknowledgement and witness your signature in Section B on the form. DO NOT complete Section B before visiting the Notary Public. You may then mail or physically deliver to our office both the completed and notarized document and a copy of your government-issued identification used to verify your identity. Electronic submission via the Secure File Upload cannot be accepted for this form.

 

Note: A Notary Public can typically be found at a local bank, credit union, insurance agency, law office, or shipping store.

If you or any of your FAFSA contributors earned income in a foreign nation, you must submit the tax information issued by the tax authority in that country.

If you or any of your FAFSA contributors indicated on your FAFSA an amount of untaxed IRA or pension distributions received in the tax year used by FAFSA, the untaxed portion of the IRA or pension distributions must be reported as untaxed income on the FAFSA, unless it was a rollover into another qualified retirement plan. If the amount was rolled over, you must provide the completed form and a copy of the IRS Form 1099-R for the appropriate tax year to verify the rolled over amount.

If you or any of your FAFSA contributors were not required to file a tax return, you may be required to verify that a tax return was not filed with the IRS. You will need to provide all Form W-2s, as well as verification of non-filing status. Instructions are provided on the forms for requesting verification from the IRS of your non-filing status.

If you or any of your FAFSA contributors did not file a tax return in the U.S., but did file a Foreign Tax Return in another country(ies), you are required to complete our form and provide a copy of the return with all tax information.
