FASI Assessments

The Functional Assessment Standardized Items (FASI) is an evaluation to help determine the services and supports needed by people with disabilities and elderly adults. The Wyoming Department of Health, Home and Community Based Services (HCBS) Section contracted with WIND to research how well the FASI can accurately identify the level of care needed for this population in Wyoming.

WIND has been conducting FASI evaluations since September 2023.  Evaluations conducted by trained assessors at WIND and took up to one hour to complete. People that had been currently served by HCBS were randomly selected to complete the FASI and will receive an email and flyer from WIND.
