Care Coordination Certificates

The courses will be delivered in a virtual environment using a combination of pre-recorded didactic presentations, asynchronous discussion threads, and a virtual meeting space for a live discussion at the end of each module.  Courses include:

Annual Wellness Visit Certificate Course

January 27, 2020 through April 3, 2020 (Registration Deadline: Jan 20th)

This course is designed to provide health professionals, such as Nurses, Health Educators, Health Coaches, and alike, with the knowledge, skills, and abilities to conduct this in-depth consultative visit and develop an individualized preventive plan of care as a valuable member of the primary care provider team.
*This course is approved for 12.5 contact hours

Care Coordination Fundamentals Certificate Course

February 3, 2020 to May 22, 2020 (Registration Deadline: Jan 27th)

This course is designed to provide health professionals, such as Nurses, primary care clinical staff and alike, to improve care coordination for patients with chronic conditions, behavioral health issues, as well as incorporating health promotion into their practice.
*This course is approved for 20 contact hours

Advance Care Planning Certificate Course

April 6, 2020 thru May 29, 2020 (Registration Deadline: March 30th)

Ensuring that patients receive the end of life care THEY want does not happen by chance. It takes planning and most importantly – it takes the skills of an Advance Care Planning professional with the ability to assist patients to put into words what is most important to them. Now more than ever it is imperative that healthcare professionals have the knowledge, skills, and abilities to hold these conversations and deliver this new benefit to the growing number of Medicare beneficiaries – currently over 55 million with an additional 10,000 plus added each day. This course will help you become a skilled Advance Care Planner.
*This course is approved for 10 contact hours

About the Instructor:

Faith Jones     HealthTechS3

Faith M Jones, MSN, RN, NEA-BC.  Director of Care Coordination and Lean Consulting at HealthTechS3

Faith Jones began her healthcare career in the US Navy over 30 years ago. She has worked in a variety of roles in clinical practice, education, management, administration, consulting, and healthcare compliance. Her knowledge and experience spans various settings including ambulance, clinics, hospitals, home care, and long term care. In her leadership roles she has been responsible for operational leadership for all clinical functions including multiple nursing specialties, pharmacy, laboratory, imaging, nutrition, therapies, as well as administrative functions related to quality management, case management, medical staff credentialing, staff education, and corporate compliance. She currently implements care coordination programs focusing on the Medicare population and teaches care coordination concepts nationally. She also holds a Green Belt in Healthcare and is a Certified Lean Instructor.
