• WSVL Accession Form - Use for all submissions except Brucellosis, Trichomoniasis, Sequencing and EIA.
• Multiple Animal ID Sheet - Intended for use as a second page to the WSVL Accession Form to detail multiple animals.
• Hard Copy EIA (Coggins) and Brucellosis forms are available from the USDA office at (307) 432-7960.  2020 Updates to the EIA Regulations
• Brucellosis Test Record and Continuation Form, Designated Surveillance Area (DSA) Map
• CWD Accession Form -Intended for use by hunters. All others please use the WSVL Accession form and any other form required by your state.
