Certificate of Insurance

Certificates of Insurance are used to provide proof of insurance coverage. An outside party may request proof that the University of Wyoming is insured or the University may request proof of insurance from a third party. Both types of requests can be submitted to Risk Management.

Contracts need to be completed in accordance to the Office of General Counsel guidelines.

Requesting Certificates of Insurance

If an outside entity requests a Certificate of Insurance, please email risk@uwyo.edu with the following information:

  • Name and Address of entity
  • Reason for request (facility rental, contract, etc.)
  • Any documentation outlining insurance requirements
