PRWORA Eligibility Form
In compliance with the new regulations, as of Dec. 1, 2025, WIND/WATR will need to receive a PRWORA eligibility form from individuals who participate in trainings, receive services, borrow equipment, or benefit from our federally funded projects. Individuals who have existing devices out on short term loan from WATR, individuals wanting to obtain new devices loans, and individuals participating in our demonstration/training events will need to complete the PRWORA Eligibility form. The PRWORA Eligibility Form will need to be downloaded from the website (see button below), saved with your information and signature, and then returned to watr@uwyo.edu. You may also use pdf version of the file. If you prefer, you may print and fax the form to us at 307-766-2763. If you fax the form, please notify us of the incoming fax by email or phone call to 307-766-6187 or 1-888-989-9463. You may also sign the form in person at the WATR lab office (College of Health Sciences, Office 151).

Wyoming Assistive Technology Resources (WATR) is looking for users of assistive technology/self-advocates, family members, and professionals who serve individuals with disabilities to join our Assistive Technology (AT) Advisory Council.
Consider joining our next Council meeting in the spring or fall! You can join through Zoom or in person at the designated location to learn about WATR and consider serving on our council.
The WATR Council members advise the WATR staff about strategies to reach and serve individuals with disibilities, families, and professionals in Wyoming. The Council also provides an opportunity for members to voice interests and views on issues related to device loans, trainings, demonstrations, research and information, and assistance. The Council meets twice a year in the spring and fall with lodging and transportation reimbursement for Council members provided.
