New Employee Deadline Information
Refer to the benefits packet provided during your new employee orientation session. The packet includes information about the retirement, health, dental, and life insurance plans available to you. Information about several optional benefits and the necessary enrollment forms are also included. All of the required forms have a RED “x” in the upper right hand corner. This indicates they must be returned to our office. These forms must be completed even if you choose to opt out of enrollment. The required forms are:
- New Benefited Employee Enrollment Check List
- Retirement Plan Option (pink form)
- Employees’ Group Insurance Benefit Application
- State of Wyoming Flexible Benefits Plan Enrollment Form

IMPORTANT: Employees must complete their forms within 31 days of the date of eligibility. Please be aware that the employee contribution for insurance is taken out in the prior month. Please submit your forms in the timeliest manner possible as to not incur a double-deduction. Following the example below, if your start date is April 13th, but you turn in your required forms on May 1st, then on payday for May, the employee contribution will be taken out twice: one to pay for your insurance for the month of May and one more time to pay for the month of June. After this point in time, you should be on track and will not be double-deducted again.
(Your date of eligibility is the first day of the month following your start date. Example: If your start date is April 13, your date of eligibility is May 1. You would then have until May 31 to enroll in your benefits.)
