W-9 Requests
Businesses may request a W-9 from the University of Wyoming before they will issue any payments. A current W-9 can be found here.
If the requester provides an alternate or substitute Form W-9, please forward the form to the Tax office, at tax@uwyo.edu or Old Main Room 101D for completion. Please include a note with your department information and whether the form should be returned to you or sent directly to the requester.
